Care Heroes AFH is owned and managed by three experienced nurses with a nursing career spanning over 15 years, including working in ICUs, long-term facilities, acute care hospitals, medical-surgical units, and hospice care. Before that, our nurses worked as Medication aides and CNAs in numerous nursing homes, adult family homes, and assisted living facilities, which instilled in them a profound understanding of the complexities of various patient care needs.

We communicate closely with our residents and their families to effectively participate in care and decision-making. We believe in fostering an environment that promotes residents' independence and preserves their dignity. Through personalized care plans and supportive interaction, we empower residents to maintain control over their lives while receiving the required assistance.
